Renew registration

ANNUAL RENEWAL OF REGISTRATION 2012
Due no later than 27th January 2012 (late payment will incur a reinstatement fee)

It is an offence under the Veterinary Practice Act, 2003, Section 36, to practise without current registration. Failure to renew registration means that your registration will lapse and your name will be removed from the Register. Annual Practising Certificates will be returned to you as soon as possible after processing. This represents your receipt for taxation purposes.

If you do not wish to renew registration in SA, please tick / circle the appropriate section and return the form. If you do not return your registration form you will not be included in the 2012 Register.

FEES:

Annual Practice fee for SA registrants – Primary (primary registration applies if your primary place of practice and residence is in SA)	$375.00
Annual Practice fee for interstate registrants – Secondary	$230.00
Copy of the Register – hard copy, inc. GST	$ 50.00
Copy of the Register – electronic copy, inc. GST	$320.00
Limited Registration (practicing, with conditions)	$375.00
Limited registration (non-practising) (to remain on Register and receive mail-outs, but will be upgrading)	$140.00
Limited registration (non-practising)/ Retired (to remain on Register and receive mail-outs)	$ 80.00
Reinstatement to Register / Late Fee	$140.00

Payment: Cheques/Money Orders payable to Veterinary Surgeons Board SA or provide credit card details. There is no GST payable on statutory fees (such as registration).
